08-09 Benefit Plan Design Changes

  Staff and Temporary Faculty Prescription Drug Plan

  • Prescription Drug Plan maximums: All three prescription plans are increasing from $10,000 or $15,000 (depending on the plan) to $25,000 annually.
  • Prescription Drug Co-pays: In addition, the appropriate co-pay based on the type of drug utilized will now apply until the maximum is reached (previously a 30% co-pay applied regardless of the drug type once the annual plan costs were within $5,000 of the maximum).
  • Step Therapy will be required for certain maintenance medications.  Additional information on step therapy is available at http://www.hrs.cmich.edu/benefits/rx-compare.html.
  • Smoking Cessation: Coverage for smoking cessation has been added to staff and temporary faculty prescription drug plans in order to help reduce costs from smoking related health conditions.  Coverage is limited to $600 annually. The participant must get authorization prior to filling the prescription by providing the Benefits Office with proof of participation in a support program for smoking cessation in order to use this coverage.  Additional information regarding the prior authorization and free support programs is available at: http://www.hrs.cmich.edu/benefits/rx-compare.html.

  CMU Dental Plan

  • This is your year to make Dental Election changes:  The CMU dental plan requires a two-year election.  This is the first year of a two-year dental election period which is your opportunity to elect the dental plan of your choosing. Elections are binding and no changes will be permitted after the conclusion of open enrollment.  Elections made during open enrollment will remain in place from July 1, 2008 through June 30, 2010, unless you experience a qualifying status change event.  For more information on status changes, visit: http://www.hrs.cmich.edu/benefits/status-changes.htm.
