Prospective Election Changes

Election Changes must be made using the status change form, which is a salary reduction agreement. Both funding and coverage for a qualifying status change must be on a prospective basis except for the retroactive enrollment that applies in the case of an election made within 30 days of a birth, adoption or placement for adoption and guardianship.

Identify if your status change is allowable under IRS law (check the site) http://www.changeofstatus.com/ and then follow the three steps outlined on the website... We have not been required to put in place all IRS changes.. some type of changes may or may not be allowed based on CMU's plan document.  Do we allow it in our plan document? - Please refer to the plan document

Certain life events
Allow you to make changes in benefits that otherwise are possible only once a year during an open enrollment period. This summary outlines the benefit plan changes you may need to make as a result of your child’s birth. Information sheets are also available at the Benefits office describing benefit decisions that may result from these other life events: adoption/legal guardianship, death of an immediate family member, divorce, ending of a spouse’s insurance, marriage, spouse obtaining insurance coverage, taking an unpaid leave of absence, and termination of employment at CMU. Life Events

  1. When you ADOPT a child or are granted LEGAL GUARDIANSHIP
  2. When you have DEATH in your immediate family
  3. When you get DIVORCED
  4. When you get MARRIED
  5. When you have a CHILD(ren)
  6. When your SPOUSE OBTAINS INSURANCE coverage
  7. When your SPOUSE LOSES his/her INSURANCE coverage
  8. When you TERMINATE your employment with CMU
  9. When you take an UNPAID LEAVE of absence
  10. When you are ready to RETIRE
