Welcome to the CMU HealthMedia® SucceedTM Log- In Page!
Before you log in to complete your HealthMedia® SucceedTM health risk questionnaire, you will want to have the following information handy:
- CMU Global ID (required) - first part of your CMU email address, i.e. chips1aa. You can find your global ID in the campus phonebook. Spouses/same-sex domestic partners should use the employee's Global Id
- CMU Campus ID Number (required) - system assigned ID number that has replaced your social security number as the primary identifier. It can be found on your pay stub or on-line remuneration statement. Spouses/same-sex domestic partners should use the employee's Campus ID Number
- Height
- Weight
- Waist Circumference (measured between the lowest rib and the top of the hips to the nearest inch) – If you need assistance with this measurement contact the CHIP facility at 3245.
- Blood Pressure
- Total Cholesterol number
- HDL Cholesterol number
- Blood Glucose level
This information will provide you with the most complete results report and personal plan possible. Don’t worry if you don’t have some of these numbers, you can still complete the HRA. You might want to consider getting this information for completing the questionnaire next year!
To complete a SucceedTM HRA for the first time:
- Go to http://www.my.healthmedia.com
- Click on New User Sign Up button
- Enter your CMU Global ID as your access code – Spouses/same-sex domestic partners should use the employees CMU Global ID
Once you have signed on and followed the process above, you will create a personal profile account. You will only have to do this once. You will be asked to create a USER name and PASSWORD that you will use from that point on to access your personal information and participate in the programs listed below. For security reasons, it is recommended that you not use your CMU global ID and CMU password as your HealthMedia® user name and password.
If you completed the SucceedTM HRA in the last 3 years:
By completing the HRA, you created a personal account and do not have to create a new one. To complete your HRA this year:
- Go to http://www.my.healthmedia.com
- Click on Returning User Log In button
- Enter your user name and password
- Click on Programs button (star)
- Under "Start a New Program" click on Health Risk Assessment.
Once you have completed your SucceedTM questionnaire, you will have the opportunity to participate in the following programs:
- HealthMedia® NourishTM, a nutrition program.
- HealthMedia® BreatheTM, a smoking cessation program.
- HealthMedia® BalanceTM, a weight management and physical activity program.
- HealthMedia® RelaxTM, a stress management program.
- HealthMedia® CareTM for Your Back, a low-back prevention and self-management program. NEW
- HealthMedia® OvercomingTM Insomnia, a six-week interactive insomnia program. NEW
To Access these Programs:
- Go to http://www.my.healthmedia.com
- Click on Member log in
- Enter your user name and password
- Under "Start A New Program " click on the program of your choice
