The CHIP rehabilitation program operates on an appointment basis to better serve our clients. Walk-ins are welcome and will be worked in around scheduled clients. Please call 774 -3198 to schedule an appointment with one of our athletic trainers.
For work-related injuries, employees may walk-in or be referred by a supervisor or physician for treatment. Preference will be given to employees who walk-in with work-related injuries.
Non work-related injuries are also welcome at the CHIP Facility. Clients must schedule an appointment and have a prescription for rehabilitation from a medical provider in order to utilize the services at CHIP.
Please call the CHIP facility at 774-3198 to schedule your first visit. You will be asked to complete a Pre-Treatment Screening form, after which your injury will be evaluated and treated. Your initial visit will take approximately 45 minutes. Normal treatment time ranges between 20-30 minutes.
